How to get the supplies you need

If you live in one of the following zip codes, your students may qualify for a backpack full of school supplies:

  • 37421 – Harrison
  • 37416 – Hwy 58/Bonny Oaks
  • 37353 – McDonald (Hamilton County Only)
  • 37363 – Ooltewah
  • 37302 – Apison
  • 37308 – Birchwood (Hamilton County Only)
  • 37315 – Collegedale
  • 37421 – East Brainerd/Tyner
  • 37336 – Georgetown (Hamilton County Only)

Stop by the Samaritan Center office or call 423.238.7777 to set up an evaluation appointment. Be sure to bring all the required paperwork (see list below), and once you have been qualified, your children can visit the Stock Their Lockers “shop” to choose the school supplies they need to succeed!

Documents Needed

  • Picture Identification
  • Social Security cards for everyone in the home
  • Proof of residency: lease, mortgage, or rental agreement
  • Proof of income from all in the household for the last 30 days
  • Letter of employment separation/unemployment
  • Proof of child support: letter or receipts
  • Letter from Social Security office: disability, SSI
  • Proof of government assistance: food stamp (SNAP) letter
  • Current bank statement
  • All current bills, paid or unpaid: electric, water, phone (cell or other), cable, AND disconnect notice
  • All other bills or receipts for anything paid out during last 30 days: household/car repairs, loans, insurance, doctor bills, prescriptions
  • If under a doctor’s care, bring a letter from doctor or medical papers
  • W-2 forms and taxes filed Jan-April 30
  • Any other requested information or paperwork
